On your first visit to Upstate Chiropractic and Wellness you will be given paperwork to fill out regarding your current complaint and past medical history. Please be sure to have your insurance card and photo ID with you as we will need to make a copy of it for your chart. You may wear loose fitting clothing for your comfort while being treated. You will be able to talk to Dr. Deyo before your treatment and get to know her philosophies and her goals for your treatment. Dr. Deyo will perform an exam to determine what the underlying factors of your condition may be, and will utilize her various techniques to tailor a treatment plan specifically for you.
